Getting started is simple. Reach out through our contact form or schedule a call—we’ll walk you through the next steps and answer any questions along the way. For Skilled Nursing services contact the office at 773-924-5900. Our team will verify your insurance coverage and eligibility. We will coordinate with your physician to obtain any required orders and documentation. A member of our clinical staff will contact you to schedule an assessment. After the assessment, services can begin and your personalized plan of care can be implemented.
